摘要:
Medical errors occur every day in the healthcare setting. The intensive care units (ICU) are at a high risk of these medical errors due to several things including the acuity of the patient and the high pace nature within these units. As errors occur a root cause analysis is performed to determine the cause of the error and then implement ways to prevent that particular error in the future. It has been observed that many errors in the ICU setting could be prevented if the overall environment in the ICU (staffing, patient acuity, workload, etc.) is assessed to provide staff with an assessment of if they are at risk of an adverse event occurring. Beth Israel Deaconess Medical Center (BIDMC), in partnership with MIT, has created an approach and algorithm to provide staff with this assessment. The Veterans Health Administration has chosen explore this approach for possible use in the ICU’s. The goal of this project is to try to replicate this approach in an effort decrease ICU adverse events to Veterans through a proactive approach.
